A cognitive screening is a quick, non-diagnostic, check of your cognitive health to determined if further testing may be needed.

What to expect

Participants will complete a brief and confidential memory screening with an experienced nurse practitioner and receive:

  • a summary of their results, which they can share with their existing care team.
  • education on modifiable dementia risk factors and signs of normal cognitive aging versus potential decline.
  • information about free and low-cost community-based resources related to brain health and aging.

Who should sign up

A memory screening is important for all older adults, not just those experiencing changes in their memory and thinking. You are invited to sign up if you:

  • Have noticed “senior moments” happening for the first time or more regularly, including lapses in memory and concentration
  • Have a family history of Alzheimer’s or a related dementia
  • Simply want peace of mind and want to be proactive about your brain health!

Please note that screenings are designed to help determine whether further evaluation may be necessary. This is not a diagnostic service. Participating in this cognitive screening does not establish a clinician-patient relationship with Sunday Health.

Will you share the results of the screening with anyone?

No, your results are fully confidential. You will receive the results via email or hardcopy, and they will not be shared with anyone else.
