How it works
During this brief call, our nurse care navigator will explain the importance of cognitive care, outline Sunday Health's care model and services, ask you to share your goals and concerns, and answer questions. The purpose of this call is to ensure our services align with your goals.
When you are ready, our nurse care navigator will help you schedule an appointment and explain the next steps in the process. Our nurse care navigator will coach you along every step of the way.
Before your initial telehealth visit, you will complete three computer-based assessments: a timed, interactive cognitive assessment and two standardized mood assessments. You will receive the results of the assessments during your first visit.
During your Brain Health Assessment visit, you will meet with our nurse practitioner to discuss your goals and concerns, review your medical history, medications, and risk factors for cognitive decline, and complete additional assessments of cognition, mood, safety, and lifestyle. The results of your Brain Health Assessment will guide your follow-up plan.
If your Brain Health Assessment is reassuring, you will meet with our nurse practitioner to develop a strength-based Brain Health Plan tailored to your goals. If your Brain Health Assessment is concerning for cognitive decline or your risk factor profile is in the moderate-to-high range, you will meet with our neurologist for in-depth testing.

We care for patients residing in the District of Columbia, Maryland, and Virginia. We provide all services via telehealth.
We currently accept Medicare Part B (also known as Original Medicare or Medicare fee-for-service). There are no extra out-of-pocket costs beyond your standard 20% coinsurance*. If you have a Medicare Supplemental Insurance (Medigap) plan, that Supplemental or Medigap plan would likely cover most or all of the 20%.
If you do not have Medicare Part B insurance, we offer self pay options which you can explore here.
*Please note that this does not include the annual deductible as part of your Medicare policy.
Get Started: Your initial visit with Sunday Health starts with a 15-minute welcome phone call with our care navigator. She will collect basic information and schedule your first video visit with one of our experienced clinicians. She will also send you links to online exercises that we will ask you to complete in advance of your first video visit.
Check Your Memory and Thinking: Our experienced clinicians will ask you a series of questions and administer various tests of your thinking and memory. Your answers and test results will help us establish a baseline of your cognitive function. This is important for future comparison and to identify any problems early.
Receive a Thorough Assessment: Our doctors and nurse practitioners will spend time discussing with you the results of their evaluation. You will not be rushed. We will take the time to understand your family's situation by asking many questions and will address all of your questions and concerns.
Help Protect Your Memory: We'll develop a personalized brain health plan with you — tailored to your goals, preferences, and ongoing needs (depending on diagnosis, if any). You’ll receive more than just “eat better,” and “exercise more often.” The plan will incorporate an evaluation of your medical history, current symptoms and medications as well as current challenges. We will offer actionable suggestions for preserving and nurturing your brain health.
