For individuals who are identified during the initial evaluation and need to be evaluated by a dementia neurologist. We will evaluate you for a potential diagnosis or intervention for something causing the symptoms that are actually reversible.
When it comes to cognitive aging, it is less “one size fits all” than “one size fits one.” A brain health baseline will allow your cognitive performance in later years to be compared to your earlier ability.

Step 1: A free call with our nurse care navigator
During this 15-minute call, you will learn about the importance of cognitive care and what you can do to support your brain health. You will hear about our clinical care model and services. You will also have the opportunity to ask questions to ensure our services match your needs.
Step 2: Book and undergo initial evaluation
After scheduling your appointment, we will walk you through the next steps including an interactive, computer-based cognitive assessment and mood-related questionnaires. The results will inform the next steps in your evaluation. You will receive the results during your visit.
Step 3: Your first telehealth visit
During your 90-minute Brain Health Assessment visit, our nurse practitioner will evaluate specific aspects of your cognitive, emotional, and overall medical health to identify any concerns or areas needing support. You will receive the results during your visit.
Step 4: Your second telehealth visit
During your 45-minute Brain Health Plan visit, our based on all test results, the nurse practitioner will connect these to final aspects of your goals and lifestyle. You will receive a written report after your visit.

During your 75-min Neuropsychological Testing visit, our neurologist will administer in-depth assessments of each cognitive domain to identify your strengths and specify areas of concern. Our neurologist will provide you with the results of your diagnostic evaluation and a written report during your next visit.
Step 5: Your third telehealth visit
During your 60-min Cognitive Care Plan visit, we will review the results of your diagnostic evaluation, explain your diagnosis, and outline your treatment plan. You will receive a written report after your visit.
