During this 15-minute call, you'll meet with our nurse care navigator to learn about the importance of cognitive care and what you can do to support your brain health. You will also have the opportunity to ask questions to ensure our services match your needs.
Once you are ready to move forward, our care team works diligently to request and gather your medical records to help ensure we have a complete picture of your health history.
After scheduling your appointment, we will walk you through the next steps including an interactive, computer-based cognitive assessment and mood-related questionnaires. The results will inform the next steps in your evaluation. You will receive the results during your visit.
During your 90-minute Brain Health Assessment visit, our nurse practitioner will evaluate specific aspects of your cognitive, emotional, and overall medical health to identify any concerns or areas needing support. You will receive the results during your visit.
Based on your needs, you will work with our nurse practitioner to develop a detailed Brain Health Plan or undergo more thorough Neuropsychological Testing with a dementia neurologist. Both are a time to connect your cognitive assessment results with your goals, lifestyle, and areas of focus to maximize your brain health as you age.

You may use your Flexible Spending Account (FSA) or Health Savings Account (HSA) to pay for your Sunday Health telehealth visits.
However, individuals are responsible for working with their FSA/HSA provider to confirm that the services fall into the plan’s allowable expenses (“qualified medical expenses”).
Your Sunday Health nurse care navigator can provide you with an itemized receipt (“superbill”) for potential insurance reimbursement for the fees associated with your telehealth visits. If you have out-of-network benefits through your insurance plan, you can submit the superbill to your insurer for potential reimbursement.
As Sunday Health cannot guarantee that your insurer will partially or fully reimburse you, we encourage you to contact the number on the back of your insurance card for guidance on your plan’s out-of-network benefits and reimbursement policies.
